Abstract

Osteoarthritis (OA) is a degenerative joint disease. The joints most commonly affected are the knees and pelvis leverage characterized by the changes of the biochemical and metabolic in the body that cause cartilage erosion followed by periarticular bone response. Age becomes the strong predictor of the disease. In Indonesia, about 50%- 60% of OA patients experienced in elders. One of OA treatments is NSAIDs. The high prevalence of multiple drug use combined with age-related changes in pharmacokinetics and pharmacodynamics makes elderly patients with OA more vulnerable to drug related problems (DRPs). This study aims to assess the drug utilization and to identify the actual or potential of DRPs from the use of NSAIDs in elderly patients with OA at Geriatric Clinic Dr. Soetomo Hospital Surabaya. It was an observational study with cross-sectional method. This study was conducted from March to May 2014. The results showed from 70 patients in OA patients was more prevalent to the elderly woman than men. Hypertension is the highest comorbid, followed by diabetes mellitus. There are four types of NSAIDs that are used which are: natrium diclofenac, mefenamic acid, meloxicam and ibuprofen. The dose regimen in this study were natrium diclofenac (1-3 x 50 mg ; 1 x 25 mg), mefenamic acid (1-3 x 500 mg), meloxicam (1 x 7,5 mg- 15 mg) and ibuprofen (1-2 x 400 mg; 3 x 200 mg). This study founds that there were four types of DRPs, a patient can experience more than one DRP, which include: unnecesarry drug (therapeutic duplication) (1,4%), inappropriate dose (18,6,2%); potential adverse drug reaction (18,6%) and potential drug interactions (25,7%).
