Abstract

Background: The maxillary labial frenum may present aesthetic problems or compromise the orthodontic result in the midline diastema cases, thus causing a recurrence after the treatment. The management of such an aberrant frenum is accomplished by performing a frenectomy with classical technique. Case: A 13-years-old female patient was referred from Department of Orthodontics for frenectomy. On examination revealed high maxillary frenum attachment with midline diastema. Patient is requesting for correction. Case management: The area of the frenum was anaesthetized by local anaesthesia. The frenum was engaged with a haemostat. Incisions were placed on the upper and the under surface of the haemostat until the haemostat was free. The triangular resected portion of the frenum with the haemostat was removed. A blunt dissection was done to relieve the fibrous attachment. The edges of the diamond shaped wound were sutured using 4-0 black silk with interrupted sutures. The area was covered with a periodontal pack. Result: One week post frenectomy showed the presence of redness and slight oedematus on the mucobuccal fold regio 11 21. At 1 month of follow-up, the healing was found with no tension at the frenum area. Conclusion: frenectomy with classical technique has successful outcome to correct the aberrant frenum.
