Abstract

Background: Mucosal damage is a result of gastrointestinal tract infection. Mechanism of regeneration mucosa after probiotic administration still poorly understood. Lactobacillus plantarum IS-10506 is a native Indonesian probiotic fermented milk originating from Sumatra. Objective: To explain the mechanism of activation, proliferation and maturation of intestinal stem cells by LIS-10506 IS, either as a preventive or curative to accelerate regeneration intestinal mucosal damage caused by LPS E. coli O55:B5. Method: Sixty-four Sprague-Dawley rats were divided into 4 groups: control group (KN); received LPS Escherichia coli O55:B5 group (KL);received LPS Escherichia coli O55:B5 then probiotics (KL-Pr) as curative group; and group received probiotics 6 days before LPS Escherichia coli O55:B5 and continued probiotics back (KPr-7L) as preventive group. Probiotics used were LIS-10506. Necropsy was done on days 3,4,6 and 7 with ileum as unit analysis. Scanning Electron Microscope examination was conducted to evaluate structure of intestinal mucosa and expression of Hsp27, IL-10, Stat5, Lgr5, Bmi1, ERK, β-catenin, Ki67, Hes1 and cytokeratin20 were evaluate with immunohistochemistry examination. The statistical analysis used was ANOVA, Kruskal Wallis A, Brown-Forsythe, Mann-Whitney and pathway analysis. Results: A significant increase is seen in LIS-10506 receiving groups on Lgr5, Bmi1, Hsp27, IL-10, intestinal stem cells activation (Stat5, ERK, β-catenin), proliferation (Ki67) and maturation (Hes1, cytokeratin20). SEM examination, Ki67 and cytokeratin20 showed that KPr-7L group showed the fastest mucosal regeneration on day 4.Pathway analysis showed that mechanism of acceleration regeneration intestinal mucosa used different pathway on curative group compared with natural pathway and preventive group used natural and alternative pathway. Conclusion: Probiotics accelerated regeneration of intestinal mucosa and the best is preventive group. Healing mechanisms in curative and preventive groups used different pathway.
