Abstract
Latar Belakang. Atresia bilier (AB) merupakan kolangiopati obstruktif yang progresif dengan mortalitas tinggi, yang diduga akibat disregulasi imun dan multiple hit mechanism yang melibatkan sel Treg (direperesentasikan oleh sel CD4+/FoxP3+) dan Th17 (CD4+/IL-17+) Tujuan. Membandingkan jumlah sel CD4+/FoxP3+ dan CD4+/IL-17+ pada bayi kolestasis dengan dan tanpa AB. Subyek dan metodologi. Sampel biopsi hati bayi kolestasis usia 1 – 12 bulan yang dirawat di instalasi rawat inap Ilmu Kesehatan Anak RSUD dr Soetomo, Surabaya selama periode 6 bulan diambil untuk dilakukan pengecatan HE untuk penentuan diagnosis AB dan non AB, serta pemeriksaan imunohistokimia (IHK) untuk penghitungan jumlah sel CD4+/FoxP3+ dan CD4+/IL-17+. Hasil. Dari 34 sampel didapatkan 20 bayi AB dan 14 bayi non-AB. Hasil pemeriksaan IHK menunjukkan jumlah sel CD4+/FoxP3+ pada kelompok AB lebih rendah dibanding nonAB (median: 6 vs. 14 sel per lapangan pandang (sel/lp); p<0,001); sedangkan jumlah sel CD4+/IL-17+ pada kelompok AB lebih tinggi dibanding pada kelompok non-AB (median: 7 vs. 5 sel/lp; p=0,04). Kesimpulan. Jumlah sel CD4+/FoxP3+ lebih rendah sedangkan jumlah sel CD4+/IL-17+ lebih tinggi pada bayi kolestasis dengan AB dibanding tanpa AB.
