Abstract

Background : In managing cases of burns injury, we are faced with a complex problem with high morbidity and mortality. The main problem in burns injury are lossing fluid, which cause unstable haemodynamic and syok hypovolemic. These unstable haemodynamic affect multiple organ function, such as kidney, lung, heart and eventually make a multiple organ dysfunction syndrome. Lactic acidosis is produced in muscle cells and red blood cells and forms when the carbohydrates are use for energy during times of low oxygen levels (anaerob metabolism). The level of lactic acid and base excess in blood is an important marker of hypoxia and also showed the adequacy of fluid rescucitation in burns. Ringer’s acetate and ringer’s lactate are crystalloid, which lactate had metabolism in liver while acetate had metabolism in muscle. Ringer’s acetate has significantly faster metabolism than ringer’s lactate, it also consume less oxygen. This research will compare the effectivity between ringer’s acetate and ringer’s lactate as a fluid rescucitation in burns injury. Method and Materials : Comparative study, using moderate-severe burn patient, rescucitation with ringer’s acetate and ringer’s lactate, measure the level of lactate acid and base excess in blood. Objective: Usage of ringer acetate decreassed amount of lactate acid and base excess in blood in moderate and severe burn patient. Results: The amount of lactate acid and base excess doesn’t give a significant result in burn patient rescucitation using ringer’s lactate and ringer’s acetate.
