Abstract
Over the past three decades, cognitive_behavioral therapy (CBT) has become o first line psychosocial treatment for individuals with chronic pain, included headache. prevalence of headache in the population is estimated 4-5%. There are many types of headache, i.e. tension_type headache, migraine, cluster headache, etc. Among diognosis of chronic headache, migraine and tension_type headache are common and can be disabling. CBT has been demonstrated to be an efficacious treatment for both disorders. specific non-pharmocologic treatment behavioral treatments i.e. relaxation, biofeedback and CBT can be used either alone or in conjunction with ongoing pharmacologic intervention. Brief cBT only S to s sessron alone or combination with biofeedback or relaxation prove to be the same efrcacy with the long session. CBT change brain neurobiological metabolism and activity effect especially cingulated cortex, prefrontal cortex and amigdale hypocampus. The function is 'top-down' regulation from hyper-exitability lymbic system is controlled by hypoactive prefrontal cortex. CBT reverse this condition. They possess the most evidence for successful headache management They have a long history of randomized trials showing effcacy, so that CBT is a gold standard therapy for pain and chronic headache and are considered first line preventive options.
