Abstract

Background Hydrocephalus in pediatric patients increases intracranial pressure. The pressure is spread by the cerebrospinal fluid through subarachnoid space, including the space under optic nerve sheath that will make changes in optic nerve sheath diameter. This pressure also causes static of axoplasmic flow in prelaminar optic nerve. Papilledema will then occur due to intrinsic edema of ganglion cell axons. The aim of this study is to analyze the correlation among optic nerve sheath diameter, intracranial pressure, and ocular fundus condition in pediatric patients with hydrocephalus. Methods This is an observational analytic study with cross section design. Hydrocephalic pediatric patients with open major fontanelle who were admitted in dr. Soetomo General Hospital underwent funduscopic examination, brain MRI evaluation, and direct measurement of intracranial pressure during ventriculoperitoneal shunt surgery. Results During the study period from July 1, 2014 to June 31, 2015 there were 10 pediatric patients. The study found that there was no correlation between right optic nerve sheath diameter, left optic nerve diameter, mean optic nerve diameter and intracranial pressure with p value of 0.987, 0.570, and 0.789, respectively. In funduscopic examination, normal fundus or no papilledema was found in all patients. Conclusions There is no correlation among optic nerve sheath diameter, intracranial pressure, and ocular fundus condition in pediatric patients with hydrocephalus.
