Abstract

Background : Congestive heart failure is a clinical syndrome characterized by neuroendocrine and cytokine activation. Activation of these substances can exert specific effects on nitric oxide synthase. Advances stages of chronic heart failure are associated with systemic activation of cytokines which have been shown to stimulate the expression of NO. Therefore, cytokine activation could lead to expression of NO in chronic heart failure. Objectives: To analyze difference plasma nitric oxide level between patients with heart failure NYHA II and NYHA IV Methods : Observational analytix research by using cross sectional. The subjects are heart failure patients NYHA II and NYHA IV in Dr. Soetomo Hospital Surabaya. Sample size are 40 patients with consecutive sampling used, consist of 20 heart failure patients with NYHA II and 20 heart failure patients with NYHA IV. Their plasma nitric oxide level is measured with ELISA method. Results: The results of Mann-Whitney U test is known that significancy 0.000 or probability under 0.01 ( p < 0.01). With the result that H0 is refused or plasma nitric oxide level in heart failure patients NYHA II different with heart failure patients NYHA IV. Conclusion : There are significant differences of plasma nitric oxide level between patients with heart failure NYHA II and NYHA IV which in patients with heart failure NYHA IV level of plasma nitric oxide are higher than patients with heart failure NYHA II.
