Abstract
Hypertension occurs in more than two-thirds of individuals after age 65. This is also the population with the lowest rates of BP control. Hipertension is an extremely common comorbidity of diabetes, affecting 20-60% of people with diabetes. Elderly diabetics often have medical, socioeconomic, or environmental factors that impact on their Diabetes. In some hypertensive diabetics, further complications arise from the resence of orthostatic hypotension, which can result from autonomic neuropathy or as a side effect of medication. Treatment decisions should, of course, be individualized based on the clinical characteristics of the patient, including comorbidities as well as tolerability, personal preference, and cost, especially for patients who must pay out of pocket for medications. combinations of agents are often reguired. when drug therapy is intensified, patients should be monitored carefuly for advcrsc effects, such as orthosratic hypotension. Finally, it is important to note that in diabetic patients the greatest reduction in cardiovascular mortality occurs at a diastolic blood pressure of - 80 mmHg. Thus, aggressive blood pressure control should be atempted in all diabetic patients.
