The cases were the person who relapse after completed rehabilitation program, while the controls were the person who still being abstinence after completed rehabilitation program. The samples were obtained using simple random sampling. Data were collected with the questionnaire and analyzed with simple logistic regression test and then multivariate logistic regression test. The result found that there are relation of addictive substance choices (p=0.029), length of addictive substance use (p=0.028) and p=0.024), craving (p=0.000), self efficacy (p=0.000), exposure of the addictive substance abuse (p=0.001), and social support (p=0.000) with the addictive substance abuse relapse, while the age, sex, education, job, marriage, living situation, and impact of addictive substance abuse had no relation with relapse. The multivariate analysis was done and the formulated index was (-3,341 + 1,832 * Self Efficacy (Low) + 1,761* Craving (High) + 1,549 * Exposure of Substance Abuse (Yes)). It can be concluded that the index can be used to screen identify the users who has high risk of relapse and give them intervention to prevent from actual relapse.
