Abstract

Introduction. CHD patients experience bio-psycho-spiritual distress and elevated expressions of VCAM-1 and MCP-1. This study aimed at explaining the mechanism of a mind-body-spiritual (MBS) nursing care influence on spiritual intelligence, Hsp70, eNOS, VCAM-1, and MCP-1 expressions. Method. This study employed a quasi-experiment design. Fourty four CHD patients treated at various rooms in three hospitals in Surabaya were divided in equal number into the control and treatment groups. The variables of interest were measure at both pre and post intervention, using questionnaires and ELISA technique with sample from venous blood serum. The control group received a standard-nursing intervention while the treatment group received the MBS nursing care for three respective days. The MBS nursing care was developed through two phases study, comprises both a qualitative phenomenology and a survey, involving a total of 158 respondents. Data collection took place from September 2016 to July 2017. Data were analysed with wilcoxon signed rank test, mann whitney u test and smart PLS. Results. The results showed that the MBS nursing care improved patients‟ spiritual intelligence (p = 0,000), attenuated the expression of VCAM-1 (p=0,028), and the expression of MCP-1 (p = 0,000), but did not have any effect on Hsp70 and eNOS. Pathway analysis showed that mind-body-spiritual nursing care improved the spiritual intelligence, which then attenuated the expression of VCAM-1 and MCP-1 through Hsp70 signaling pathway. Conclusion. It can be concluded that mind-body-spiritual nursing care plays an important role in improving spiritual intelligence and thus the expression of VCAM-1 and MCP-1. The new findings of this study is: the spiritual intelligence, which enhanced by the MBS nursing care, has a potential to control the expression VCAM-1 dan MCP-1 trough Hsp70 pathway. Further study should investigate confounding factors affecting the expression of Hsp70 and eNOS in a more robust design
