Abstract

Closed fracture generally have low risk in developing infection and the risk will be higher during surgery. Surgery in this case is categorized as clean surgery and potential bacteria to develope surgical site infection (SSI) is skin flora organisms, including Gram-positive bacteria. Cefazolin is the first generation of cephalosporin with high activity in Gram-positive coccus and very lipophyl, and those makes it has greater penetration to bone compared to other cephalosporin. The study was conducted to identify the using of Cefazolin in closed fracture patients at Universitas Airlangga Hospital Surabaya and link it with clinical data. It was a retrospective descriptive study using patient’s medical record in 2nd January 2014 – 31st December 2017 period. The inclusion criteria were closed fracture patient orthopaedic cases who had been treated by Cefazolin. Cefazolin was used single only in 89 patients (100%). The most frequent regimentation dose for prophylaxis was Cefazolin (1x2 g) IV (82 patients, 91%); The most frequent switch pattern was Cefazolin (1x2 g) IV (as prophylaxis) continued by Cefazolin (3x1 g) IV (74 patients, 83%). The most frequent administration time of Cefazolin as prophylaxis was ≤1 hour prior incision (55 patients, 62%); The most frequent duration of Cefazolin therapy post OP was not admitted or not more than 24 hours post OP (45 patients, 51%). The results are mostly already appropriate according to guidelines.
