Abstract

Hazard identification and risk assessment are parts of requirements in the risk management that has the objective of managing the aspects of occupational safety and health in attempting to create the safe, healthy, efficient and productive workplace and also as well as to fulfill the criteria set by OHSAS 18001. The main aim of this research was to carry out hazard identification and risk assessment in a workshop of heavy equipment of PT. United Tractors Tbk. Surabaya. This was an observation study with cross sectional design. The object of this study were hazard identification and risk assessment in a workshop of heavy equipment of PT. United Tractors Tbk. Surabaya. The method used in assessing risk referred to method applied by the company. Data were obtained by interviewing EHS Officer, Branch Instructor, Supervisor, Mechanic, and observing the worksite directly. The data collected were analyzed descriptively. The results of this study showed that there were 85 potential hazards, and 122 pure risk. The results of risk assessment indicated that there were 36 category of low risk, 74 category of moderate risk, 9 category of high risk and 3 category of extreme risk. It can be concluded that the each Department Head has carry out hazard identification and risk assessment on the overall process activities. It is recommended that the company provide risk assessment training to all Department Head.
