Abstract

Background. The goal therapy of ISDN is vasodilator effects produce hemodynamic consequences that can be put to therapeutic use. Owing to a decrease in both venous return (preload) and arterial afterload, cardiac work is decreased. As a result, the cardiac oxygen balance improves. Spasmodic constriction of larger coronary vessels (coronary spasm) is prevented. Objective. To find out the total number, route, and dose of the ISDN uses for patients with AMI and the possibility to find its DRPs as well. Method. The observational research and data collection technique were using cross sectional method in wich the medical record data to the patients were conducted from March 15th to June 30th. Then the results were compared with the ESC and ACC AHA Guidelines. Result and Discussion. The administration of ISDN 5mg by sublingual route was the most widely administered initial dose to AMI patients with chest pain (37,03%), while the DRPs found were the sublingual route which had to be crushed first, the suitability of a less appropriate ISDN dose, the potential seide effects such as dissiness, with no decrease of sistolic and diastolic blood pressure and no significant increase of heart rate (p>0,05) as well, and drug interaction. Conclusion. Drug Utilization Study organic nitrate preparations at AMI in RSUD Dr. Soetomo Surabaya were appropriate according to ESC dan ACC AHA guidelines.
