Abstract

Chronic kidney disease (CKD) defined as the presence of kidney damage or decreased glomerular filtration rate (GFR) for 3 months or more. One of the major causes of death in CKD patients is infection. In stage 5 CKD GFR rate < 15 ml/minute/1.73m2, patients should receive hemodialysis therapy. The aim of this study was to describe the use of antibiotic therapy in CKD patient who received hemodialysis therapy at Internal Medicine Wards of Dr. Soetomo Hospital, Surabaya during 1st March until 31th May 2012 by evaluating the drug of choice, duration of therapy, route of administration and problems related to antibiotic. This study is a prospective study. The result showed that, from 43 patients urinary tract infection is the most common infection in CKD-HD patients (48,84%). Antibiotic used were cephalosporins (90,70%), fluoroqinolones (51,16%), carbapenems (23,26%), penicillins (6,99%), metronidazole (4,65%), and macrolide (2,33%). Antibiotic related problem in this study found there were 6 dialyzed antibiotics from 10 antibiotic which is used before hemodialysis session, 31,58% were differ from dose recommendation, and there was one case of drug interaction
