Abstract

Acute myocardial infarction is defined as myocardial cell death due to prolonged myocardial ischemia. Statin promote reduction in plasma level of LDL-C and stabilize plaque by increasing the collagen content and inhibiting metalloprotease. The aims of this study were to identify the use of simvastatin in patients with acute myocardial infarction and drug related problems (DRPs). The analysis was conducted descriptively by using patients’ medical record (cross sectional) who were hospitalized during the period 15th March to 15th June 2012 at ICCU and Myocardial Installation of Hospitalization Dr. Soetomo General Hospital Surabaya. 20 mg of simvastatin were the most given dose to patients with acute myocardial infarction. Increase of dose was seen in 3 patients which become 40 mg. Drug related problems which were identified in this study are dosage too low (11 patients), potential drug interactions (with amiodaron, diltiazem, amlodipin and digoxin, experienced by 18 patients), and potential side effects (myopathy and increased liver enzymes) that must be monitored after treatment. Signs of myopathy are not found and persistence of transaminase elevation cannot be observed during this study.
