Abstract

Background: A communicable disease that still being a problem for public health is dengue viral infection. Severe dengue infection can cause rapid death especially in children if not detected early and managed properly. Based on WHO 2009 guideline, Severe dengue infection is characterized by one or more criteria: severe plasma leakage, massive bleeding, and organ involvement. Epidemiological picture of patient with severe dengue infection is important to anticipate the potential of worsening and manage dengue infection patients. Objective: To determine the epidemiological picture of children with severe dengue infection in Dr. Soetomo General Hospital. Method: This is a descriptive-observational study with cross sectional design which used secondary data from medical records. From 135 patients with Dengue Hemorrhagic Fever (DHF) aged ≤18 years, 48 children with severe dengue infection were hospitalized in March – April 2019 in pediatric ward of Dr. Soetomo General Hospital. This study is using purposive sampling and the data are analyzed with Microsoft Excel. Nutritional status is assessed with weight for height/length graphic cut off z-scores from WHO 2006 for children aged 0–5 years old and ideal body weight percentage according to Waterlow criteria for children aged >5–18 with reference CDC 2000 graphic. Result: The characteristics of children with severe dengue infection comprise of children >5–12 years old (72,9%), male (58,3%), live in Surabaya (89,6%), currently studying at elementary school (60,3%), from other health service referral (81,3%), hospitalized for ≤5 days (66,7%), day of illness (day of fever before hospitalized) ≥4 days (89,6%), have moderate malnutrition (35,4%), and diagnosed with DHF grade III (73%) and DHF grade IV (27%) (Dengue Shock Syndrome) which is characterized with severe plasma leakage. There is no severe dengue infection patient with massive bleeding and organ involvement. Most of the patients are released from hospital in living condition (95,8%) and 2 patients died. No patient found with congenital disease and malignancy.
