Abstract

Introduction: Seizure is the top 2 among 10 pediatric neurologic complaints in primary care according to pediatric survey data. Benzodiazepines are considered as the drugs of choice for the treatment of seizures. The use of intravenous access is difficult and requires more time, therefore other access is considered to be more efficient, i.e. intramuscular. Midazolam has the physical-chemical characteristics that facilitate rapid absorption following intramuscular injection. Aims: To determine the effectiveness of intramuscular midazolam compared to rectal diazepam in seizures cessation in children. Methods: A clinical trial on children presented with acute seizures in emergency department and inpatient ward Dr. Soetomo Hospital, Surabaya, October 2014-April 2015. Subjects were devided into 2 groups with either received intramuscular midazolam or rectal diazepam for seizures cessation. The interval time of drug administration to cease seizure was compared. Log rank analysis was used for statistical analysis. Side effects of both drugs were evaluated. Log rank analysis were used for statistical analysis. Results: There were 66 patients, 33 in each group enrolled the study. The median time interval for seizures cessation with intramuscular midazolam was 45 seconds, significantly different compared to rectal diazepam group (180 seconds). No significant side effects seen in both groups. Discussion: Intramuscular midazolam is effective to terminate seizures in children. It can be used as an alternative treatment for acute seizure in patients with intravenous access or rectal route difficulties.
