Abstract

Background - Indonesia is one of 30 countries with the high burden of MDR-TB. It is estimated that there are 23,000 cases in Indonesia. Treatment of MDR-TB has many chalenges, one of which is the high rate of drug side effects due to the treatment given. In may 2016, WHO issued a recommendation for the use of a standardized 9-11 months short-term therapy regimen. Drug side effect that can occure from this treatment is the prolongation of QTc interval due to the administration of moxifloxacin (Mfx) and clofazimine (Cfz). Objective - To analyse the incidence of QTc interval prolongation and to study the correlation between age, sex, dose, potassium levels, blood sugar levels and albumin levels as risk factors for prolonged QTc interval in MDR-TB patients who received short-term therapy regimen. Method – This study was an observational retrospective study with a total sampling method, wich was conducted at Jember Chest Hospital. Samples from this study were patients diagnosed with MDR-TB at Jember Chest Hospital in the period of June 2018 - December 2019 who used the short-term therapy regimen and met the inclusion criteria. Results – 74 patients were included in this study. There was 29,43±48,64 ms QTc interval changes from baseline, the maximum QTc interval was 756 ms. 52 patients experienced prolonged QTc interval, mostly occurred 1 month after treatment. Grade 3 side effects occured in 17 patients. Total incidence of QTc prolongation occured in 196 of 410 studied ECG. Potassium level was the risk factor that had a correlation with QTc interval prolongation (r = -0.36, P <0.05). Conclusion - There has been a prolonged QTc interval in MDR-TB patients who received moxifloxacin and clofazimine in short-term therapy regimen, hypokalemia is a risk factor for QTc interval prolongation.
