Abstract

Diphtheria in Bangkalan District was the highest in East Java. Until the end of December 2019, there have been 34 times diphtheria outbreaks in 14 subdistricts and 28 villages (CFR 6.89%). Complete basic immunization coverage in Bangkalan Disrict for the past 4 years has not met the target and was unevenly distributed. The complete basic immunization coverage in 2019 was 70.8% of the target 93%. The aim of the study was to analyze the effect of immunization factors on diphtheria outbreaks in Bangkalan District, 2019. This study desaign was case control using secondary data from Evaluation of Village Immunization Implementation in Bangkalan, 2020. The case population was the village that experienced diphtheria outbreak in 2019, while the control population was the village that did not experience outbreak of diphtheria during 2017-2019. Respondents were village midwives and mothers under five years of age. Villages were selected using simple random sampling, while mothers were selected using a Rapid Survey method. The selected sample was 54 villages and 378 children under five. Data analysis using simple logistic regression test and multiple regression test. The results showed that the independent variables that had a relationship with Diphtheria outbreaks were the working period of the village midwife (p = 0.000), the residence of the village midwife (p = 0.025), data collection on immunization targets (p = 0.000), quality of immunization services (p. = 0.026), and cross-sector support (p = 0.016). The final result of multiple regression analysis obtained two indicators of risk of diphtheria outbreaks were working period of village midwives and the quality of immunization services. The conclusion were working period of the village midwife and the quality of immunization services in the village has an effect on Diphtheria outbreaks in Bangkalan District. Strengthening the role of village midwife by involving others sector, and establish a community empowerment program such as My Home My Village is necessary.
