Abstract

Back ground : Malignancy of head and neck are the sixth most common cancers worldwide, and squamous cell carcinomas predominate ( 90%) among malignancies of the upper aerodigestive tract. Epidemiological studies provided the first evidence that the COX-2 may be involved in the pathogenesis of malignancy and overexpressed in any kind of malignancies Objective : To find whether COX-2 is overexpressed in head and neck squamous cell carcinoma and how is the correlation between the expression of COX-2 with the histopathological grading in head and neck squamous cell carcinoma. Method : This is a cross sectional study. Samples are all paraffin blocks of head and neck squamous cell carcinoma patients stored in Department of Pathology Anatomy Dr.Soetomo Hospital Surabaya, in the year of 2004-2005, with any histopathological grading from well differentiated, moderately differentiated, poorly differentiatead/undifferentiated. All the 30 paraffm blocks are stained with COX-2 immunohistochemistry. Subjects are documented from the medical record in Head and Neck Division, Department of Surgery Dr. Soetomo Hospital Surabaya. Results : From the 30 samples, 28 (93,3%) showed positive COX-2 expression and 2 (6,7%) without COX-2 expression in immunohistochemistry stained. Regarding the histopathologic grading, well differentiated showed positive immunostained in 80% with weak intensity (1 +), moderately differentiated showed 66,7% positive immunostained with moderate intensity (2+ ), and poorly differentiated/undifferentiated showed 72,7% positive immunostained with strong intensity (3+). We found there was significant correlation between histopathological grading with intensity of COX-2 expression ( p=::; 0.0001) and the worse the cell differentiation, the stronger the expression of COX-2 intensity ( correlation coefficient 0,855 ). Conclusion : Cyclooxygenase-2 (COX-2) is overexpressed in head and neck squamous cell carcinomas (93,7%). And there is correlation between histopatological grading of head and neck squamous cell carcinomas with COX-2 expression intensity. The worse the cell differentiation, the stronger the expression of COX-2 intensity (85,5%).
