Abstract

Bum injury often occurs in daily living and potentially causes major morbidity and mortality higher than other injury. Silver sulfadiazine is antimicrobial preparations that are often given for prevention of infection in a burn lesion which is relatively narrow (5-20%). Some research showed silver components toxic to host cells, especially fibroblasts and keratinocytes. Both of these cells play a role in wound healing process. The role of the laser on burn healing process has been controversial. The aim of this study was to demonstrate the increasing of epidermal growth factor (EGF) and fibroblast growth factor (FGF) secretion of the macrophages due to the effect of the laser on second-degree burn healing process in BALB/c strain male mice. Second-degree bum lesion was created with a standard buming procedure by applying a heating plate on the backside of each of 24 BALB/c mice. 24 mice were divided into three groups randomly. Group I (K), burn lesion was treated with 0.9% NaCI solution and closed dressing, the second group (P 1) burn lesion was treated with topical silver sulfadiazine 1% and closed dressing, and group III (P2) burn lesion was treated with 4 J/cm2 laser irradiation, topical silver sulfadiazine 1% and closed dressing. A biopsy was performed on the lesion on the fourth day after the burns. We studied the effect of low level laser therapy and topical silver sulfadiazine to EGF-secreting macrophages and FGF-secreting macrophages on burn healing process by making observations on Hematoxylin Eosin staining and immunohistochemistry techniques. Statistical analysis showed that the FGF-secreting macrophages and EGF-secreting macrophages in the group which given the combination of low level laser therapy and topical silver sulfadiazine significantly higher than the 2 other groups. It was concluded that low level laser therapy induced the healing process of the second¬degree burns which was given topical silver sulfadiazine by increasing the secretion of FGF and EGF by macrophages.
