Abstract

Gene mutation of PfDHFR codon 108 was a preliminary mutation of pyrimehamine. The sequenced DHFR originating from the sensitive pyrimehamine clone 3D7 and other isolates of various degrees of resistance indicated that Ser-10S in the sensitive clone of 3D7 has changed into Asn-10S in the resistant isolates, thereby making Ser-10S-Asn a key to resistance mutation of pyrimethamine. The present research was of explorative by using consecutive sampling in several Public Health Centers of West Lombok Regency. Sampling criteria were patients with clinical malarial symptoms microscopically positive of P. falciparum. Method of species determination was of molecular by using nested-PCR of Qiagen originating from blood spots on filter papers (Whatmann 3 mm) followed by polymerase chain reaction-restriction fragment length polymorphism (pCR-RFLP). Of S2 samples, 76 were malaria-pOsitive and identified as P falciparum as was 61 samples. With regard to patients' characteristics, on the basis of sex, 59% was male and 41% was female. In terms of age cohort, 39.3% was children «15 yr) and 60.7% was adult. Most natives developed malaria (91 .S%). Average parasitic density 10.990 per ,...1 of blood. Determination of gene mutation of PfDHFR codon lOS showed 54 mutations, all of which being coded by asparagine (Asn-IOS). There was no wild type that was coded by both serine and the other mutant type of threonine. Mutation of Asn-lOS was 100% (at 54 sample) found, possibly a mutation occurred in pyrimethamine. Further investigation was needed on the other DHFR codons and/or DHPS codons and on the efficacy of treatment with sulphodoxine pyrimethamine (SP).
