Abstract

Discharge planning is essential to be implemented. The purpose of discharge platming is to prepare the patients and their families to go back home. The success of DM management is not only affected by knowledge quality and self-care, but also by the patient's compliance to medications, diet, and activities. One of the efforts to improve patient's compliance is by counseling. This study was performed with statistical analysis undergone using purposive sampling to 30 Diabetes Mellitus patients ( 15 people who get a discharge planning module using supportive education method and 15 people who do not get the module at Dr. Ramelan Navy Hospital, Surabaya. This study was performed to identify the effect of discharge planning module with Supportive Education method on Diabetes Mellitus patients, in which the evaluated aspects were knowledge, self-care, and compliance. The design used in this study was Quasi Experimental (non randomized control group pretest posttest) design. The statistic results using independent sample t test could be concluded that the significant p < 0,05 (p= 0.000) which means the hypothesis is received and there is a significant knowledge improvement to the group who gets a discharge planning module using suppotiive education method than the group who does not get it. The role of the nurse as a discharge planner is to give support with action, guidance, education and let the environment improve the knowledge, obidience and apply it into daily life.
