Abstract

Circumcision is an integral part of the Moslem ritual, which is also associated with personal hygiene. All patients in these two groups are Moslems and in their primary school years. The objective of this research is to compare between pain responses in topical anesthetic application in the form of mixed cream containing 25 mg/dl lidocaine and 25 mg/di prilocaine, and in block infiltration anesthesia, during circumcision. The other objective is to compare between complications arising from topical anesthetic application with the above-mentioned composition and from block infiltration anesthesia. The research population consists of all patients, aged 7 — 13 years old, who visited Dr. Soetomo Hospital Surabaya to undergo circumcision, gave their consent to be included in the research and who did not suffer from any anatomic anomaly and disease of the penis. The research design is an experimental study which is a non-blinded¬between-group design, where both intervention and control groups were aware of the performed procedures. The result of the research demonstrates that at the time of anesthetic administration, there was a difference in pain sensations between the group treated with block infiltration anesthesia and the group treated with topical anesthetic cream, with a significance level of 0,000 indicating that topical anesthetic cream gives more comfort and is more friendly to the patient. There was a difference in pain response in the group treated with block anesthesia infiltration compared to that in the group applied with topical anesthetic cream, with a significance level of 0,028, indicating that block infiltration anesthesia is superior in reducing pain perception during circumcision.
