Abstract

Fibrinogen is one of the important diabetes associating independent risk factor for atherosclerotic cardiovascular disease. Although hyperfibrinogenemia among Type 2 DM patients was established, the mechanism of the increasing fibrinogen level among Type 2 DM patients has not yet been completely explained. The objective of this study is to evaluate the effect of insulin resistance, blood glucose, and interleukin-6 on plasma fibrinogen level; and the effect of blood glucose on serum interleukin-6 level To answer the purpose an experimental pretest-posttest control group design study on 36 newly diagnosed Type 2 DM patients and 18 control nonmal subjects had been conducted. Additional 20 well known Type 2 DM patients was included in this study to prove the effect of blood glucose on serum interleukin-6 level. The study was held at Diabetes Outpatients Clinic, Sanglah General Hospital since 1998-1999. All subjects were observed for 12 weeks among newly diagnosed Type 2 DM and for 4-12 weeks among well known Type 2 DM. The main variables were measured in this study were blood glucose, HbAlc, interleukin-6, insulin resistance and plasma fibrinogen. Results of this study were as follows: (1) among newly diagnosed Type 2 DM patient, the mean of tasting blood glucose, 2 hour post prandial blood glucose, HbA1c, interleukin-6, insulin resistance and fibrinogen level were found higher significantly than those in normal subjects; (2) among newly diab'11osis Type 2 DM patients, there were positive correlation between plasma fibrinogen level and sex, total cholesterol, LDLcholesterol, fasting blood glucose, serum interleukin-6; (3) among Type 2 DM patients, there was effect of insulin resistance (R2= 0.563, p=O.020), fasting blood glucose (R2= 0.360, p=0.039), not t; HbAlc, and serum interleukin-6 (R2=0.513, p=O.009) on plasma tibrinogen levd; and was effect of fasting blood glucose (R2=O.319, p~O.OOI) and HbAlc level (R2=0.100, p=O.OIS) on serum interleukin-6level. Of this study could be concluded that among Type 2 DM, effect of insulin resistance, fasting blood glucose and serum interleukin-6 level on plasma fibrinogen level; and effect of fasting blood glucose and HbA 1 c on serum interleukin-6 level were proved. EfTect of fasting blood glucoseon plasma fibrinogen was seen without mediated by insulin resistance and interleukin-6. Of the results of this study we suggest that fibrinogen, a independent risk factor of atherosclerotic cardiovascular disease, is necessery measured because its level frequently increase among Type 2 DM patients; and hypoglycemic agent with adequate improve insulin resistance is needed in management the diabetic patients because actually insulin resistance has many undesired clinical implications.
