Abstract

Background: Chronic Obstructive Pulmonary Disease (COPD) is a chronic lung disease that can be prevented and treated, in the form of abnormalities in the small airways of the lungs that cause airflow limitation in and out of the lungs. Corticosteroid therapy in COPD is used short-term systemically for acute exacerbations and inhalation therapy for stable COPD. Purpose: To determine the effectiveness of the use of corticosteroids in COPD inpatients at Bhayangkara Surabaya H.S Samsoeri Mertojoso Hospital. Method: The method used in this research is observational with retrospective data collection using patient medical records on March 1, 2018 – March 31, 2022. The inclusion criteria is hospitalized patients diagnosed with COPD and receiving corticosteroid therapy. The exclusion criteria are patients diagnosed with Covid-19 and COPD patients with arrhythmia complications. Results: Corticosteroids were used as single therapy in 21 patients (64%) and in combination in 12 patients (36%). Most single therapy was methylprednisolone in 13 patients, while the most combination therapy was methylprednisolone iv or po and budesonide inhalation in 8 patients. There was an overdose of the intravenous corticosteroid methylprednisolone which was given with the oral route. The respiration rate reached the target in 29 patients (88%), and oxygen saturation in 33 patients (100%). Conclusion: the use of corticosteroids such as methylprednisolone, budesonide, and dexamethasone gave effective results (normal respiratory rate and oxygen saturation) for the treatment of COPD inpatients, and the most widely used therapy was methylprednisolone (3 x 62.5 mg) iv.
