Abstract
Background: Along with the aging process in geriatric patients, anatomical and functional changes occur in many organ systems of the human body. In addition, this condition also gives rise to various diseases, including impaired kidney function. Potentially inappropriate medications (PIMs) are risk factors that trigger drug side effects. Thus, monitoring of PIMs to ensure patient safety should be considered, especially in the emergency department. Method: In this study, PIMs analysis was performed on geriatric patients with impaired renal function who were in the emergency department. A crosssectional study was conducted on patients aged > 60 years with a history of impaired renal function (GFR value ? 80 mL/min) with or without other comorbidities and receiving at least one drug therapy in the emergency department of Universitas Airlangga Hospital. PIMs analysis was carried out based on the 2019 Beers Criteria. Results: A total of 396 geriatric patients were included in this study in which 176 (44.4%) patients received polypharmacy (? 5 drugs), 187 (47.2%) patients received 2-4 drugs, and 33 (8.3 %) patients received only one drug. Furthermore, 148 (37.4%) patients received at least one PIMs. Types of drugs that are included in the PIMs criteria received by geriatric patients with impaired renal function include ranitidine, spironolactone, fondaparinux, ciprofloxacin, tramadol, enoxaparin, and gabapentin. Conclusion: Overall, a high percentage of PIMs was found in geriatric patients with impaired renal function at the hospital's emergency department. Increased attention and vigilance for pharmacists and other professionals are needed to minimize the risk of PIMs.
