Abstract

Background: Sarcopenia is commonly associated with aging and older people, but nowadays sarcopenia occurs at a younger age. Sarcopenia in older population associated with falls, functional decline, loss of independence, increased mortality, and poor quality of life. Sarcopenia found in patients with chronic kidney disease and correlated with morbidity and mortality. Decreased of GH/IGF-1 plays a key role in the loss of skeletal muscle mass. Studies show that age, IGF-1 levels, chronic kidney disease, and chronic hemodialysis are causes of sarcopenia. This study will analyze the relationship between serum IGF-1 status (by age group) and the degree of sarcopenia in chronic hemodialysis patients at Dr. Soetomo General Hospital Surabaya.. Objective: To analyze the relationship of serum IGF-1 status (by age group) with sarcopenia in elderly end stage renal disease undergoing maintenance hemodialysis. Method: This study is an analytic observational cross sectional study in patients aged 40 years undergoing chronic hemodialysis in the hemodialysis unit of Dr. Soetomo General Hospital Surabaya in 2021 and meets the inclusion and exclusion criteria. Serum IGF-1 levels were examined using the ELISA method using a calibrated Rayto device and examined for sarcopenia status based on the AWGS criteria. The data were then analyzed using the Spearmann test. Results: From a total of 70 research subjects, the proportion of normal subjects was 4.3%, presarkopenia 8.6%, sarcopenia 24.3%, and severe sarcopenia 62.8%. The highest frequency of low serum IGF-1 status was found in the age range of 60-69 years, namely 34.3%, the lowest value of serum IGF-1 was 19.27 ng/ml, the highest serum IGF-1 value was 176.68 ng/ml. There is an association between serum IGF-1 status (by age group) and the degree of sarcopenia. Conclusion: There is an association between serum IGF-1 status (by age group) and the degree of sarcopenia in chronic hemodialysis patients.
