Abstract
Breast cancer is the most common cancer in women worldwide. Combination of anthracyclines with cyclophosphamide (AC) has been commonly used for chemotherapy in breast cancer and classified as highly emetogenic chemotherapy (HEC) according to guidelines for antiemetics. Antiemetics to treat chemotherapy-induced nausea and vomiting (CINV) have been widely developed and mostly given as prevention. Most patients receiving chemotherapy still experience nausea and vomiting despite efforts to optimize antiemetic therapy. This study aimed to analyze the incidence of acute and delayed nausea and vomiting with the use of antiemetics in breast cancer patients with various chemotherapy regimens. This study was prospective observational study conducted in June-July 2022 with total sample of 41 visits from 13 patients at Instalasi Onkologi Terpadu Universitas Airlangga Hospital. The inclusion criteria were adult patients >18 years old with breast cancer; patients received highly emetogenic chemotherapy the first to third cycle; patient received premedication and post-chemotherapy antiemetics. Results showed that the incidence of nausea on the TAC protocol was higher (45.8%) and more severe, compared to the TC protocol. In addition, delayed-phase nausea was higher on the TAC protocol (62.5%). Vomiting on the TAC protocol was also higher (25.0%) but not more severe than the TC protocol and delayed-phase vomiting was higher on the TAC regimen (12.5%). With complete control parameters, >75% of patients on both protocols met the overall criteria. There was no relationship between chemotherapy cycles and the severity of nausea and vomiting, both on the TC and TAC protocols.
