Abstract

BACKGROUND - Drug related problems (DRP) have substantial effects on human health and economic status. DRP could worsen the health and quality of life of patients to some extent and can lead to significant waste of health care resources. Clinical pharmacy services could help in identify DRPs, resolve DRPs, prevent ADEs, and reduce health care costs. OBJECTIVE - This study aimed to identify the drug related problems, clinical pharmacist’s recommendations, and implementation of clinical pharmacist’s recommendations in the prevention and/or resolution of drug related problems in chronic disease patients with polypharmacy and to analyze the cost avoidance of prevention and/or resolution drug related problems performed by clinical pharmacist on chronic disease patients with polypharmacy. METHOD - The method of this study was prospective observation with inclusion criterias were: diagnosed with chronic disease, received polypharmacy therapy, and being treated for at least 3 days. The study was carried out in the Inpatient Installation of Airlangga University Hospital in June – July 2022. RESULT AND CONCLUSION - This study found 709 potential DRPs and 64 actual DRPs in 99.1% of patients with the average of DRPs 7-8 DRPs per patient. The main DRP was related to medication safety, which was adverse drug events (possibly) occur, with the most common causes were other causes, which were potential adverse drug reactions, actual adverse drug reactions, drug interactions, and inadequate therapy. The main clinical pharmacy recommendations were at the patient level in the form of counseling to patients regarding drugs. The 98.2% of clinical pharmacist’s recommendations were accepted with 97.7% of recommendations were accepted and fully implemented. Based on the clinical pharmacist’s implementation, 97.7% of DRP status was completely resolved and 2.3% of the DRP status was unknown. Cost avoidance of clinical pharmacy services for chronic disease patients with polypharmacy were 19,907,790 IDR per month and 238,893,480 IDR per year.
