Abstract

Background: Dental caries continue to represent a major problem which, if left untreated, will cause irreversible pulpitis. Root canal treatment constitutes one potential treatment intended to preserve teeth afflicted with irreversible pulpitis. During root canal treatment, pain or swelling, referred to as flare-up, can occur at any point in the process. Objective: To analyze the occurrence of immunohistochemical flares up in vital dental pulp tissue with pulp tissue extirpation and with LPS administration followed by pulp tissue extirpation through a neuroimmunological approach based on CGRP, NaV-1.7 expression on nerve cells and HSP-70 expression, TNF- α in macrophages and caspase-3 cells in macrophages and nerve cells. Material and Methods: This laboratory experimental study was conducted using 15 Spraque Dawley rats as subjects which were divided into three groups of five subjects: a control group, a pulp tissue extirpation group and an LPS induction followed by extirpation of pulp tissue group. Test samples were collected from the apical field of the mandibular incisor and subsequently examined using immunehistochemical methods. Results: There were increasing and significantly differences in CGRP, NaV-1.7, HSP-70, TNF- and caspase-3 expression between treatment and control group (p<0,05), whereas in group with LPS intervention were identified marked increase in CGRP and HSP-70 rather than NaV-1.7, TNF- and caspase-3. Conclusions: The extirpation of the vital dental pulp tissue will induce flare up thourgh mechanism of increasing CGRP, NaV-1.7, HSP-70, TNF- and caspase-3.
