Abstract
The most accident in indonesia was repeatly accident cases. One of effort to prevent repeatly accident was accident investigation. Accident investigation was one of implementation to investigate, analyze, and prevent accident in order to not let repeatly accident occur again in the next time. Highly repeatly accident cases in PT. Kertas Leces (persero) showed accident investigation system was low. This research aimed to evaluate Accident investigation system according to International Safety Rating System (ISRS) in PT. Kertas Leces (persero). This research was used observasional method, interview, and document studies based on the ISRS. Subjek in this research was accident investigation system in PT. Kertas Leces (persero). The result showed that the fulfillment level of the subelement of accident investigation system was 73%, subelement of operating management participation was 87%, subelement of major and high-potential accident was 100%, subelement of remedial action and follow-up was 92%, subelement of incident (near-miss) reporting and investigation was 0%, and subelement of accident report maintenance was 100% . The conclusion in this research was that, the overall of accident fulfillment level based on ISRS was 76%, that mean the implementation of accident investigation system was good but management side should be concern about the accident investigation system. The lowest level fulfillment was sub element of incident (near-miss) reporting and investigation which was 0%, that mean it did’t work at all. Investigation team and management side of PT. Kertas Leces (persero) should be concern about incident (near-miss) to follow-up the accident as soon as possible.
