Abstract
Background - Acute coronary syndrome (ACS) is a term that includes all clinical syndromes compatible with acute myocardial ischemia resulting rom an imbalance between myocardial oxygen demand and supply, most often cause by atherosclerosis of the coronary. Anticoagulant has mechanism to inhibit thrombin generation and/or activity thereby reducing thrombus-related events. Fondaparinux is an indirect-acting specific inhibitor of factor Xa that has recently been studied in the setting of ACS. Objectives - This aim of the study was to analyze the utilizing and identify drug related problems (DRPs) of fondaparinux on acute coronary syndrome patient in Dr. Soetomo Teaching Hospital Surabaya. Methods - The study retrospective using data from 30 patient’s medical records during the period January 1st 2014 – December 31st 2014 and was analyzed descriptively. Results – Fondaparinux was administered once daily subcutaneous at a dose 2.5 mg in all patients with duration of treatment was 5 days. There were 2 patients who received fondaparinux have switch-therapy to enoxaparin and one patient received combination of 3 anticoagulants (fondaparinux, warfarin, and heparin). Drug related problems (DRPs) that found in this study were potential adverse drug reaction, like bleeding and interaction with antiplatelet (aspirin and clopidogrel) and fibrinolytic. This DRPs are found in 3 patients who received fondaparinux in this research that caused hematuria (3%), melena (3%), and hematemesis melena (3%). From 30 patients, only 11 patients whose known their aPTT and PT value. Out of all 11 patients there was only one who got elongation of PT and INR. Conclusion – Administration of fondaparinux in acute coronary syndrome patient in Dr. Soetomo Teaching Hospital were according to American College of Cardiology - American Heart Association (ACC/AHA) Guidelines Recommendation for NSTEMI patients but not to STEMI patients and duration of treatment were according to Dr. Soetomo Teaching Hospital Diagnose and Therapy Guideline
