Abstract

Patients with chronic kidney disease (CKD) (mainly stage 4 and 5) may develop hypoalbuminemia due to malnutrition and/or albuminuria. Albumin as a protein coloid has been used for years in the treatment of patients with hypoalbuminemia. Albumin is a plasma expander which is most expensive compared to others. Up to this day, the use of albumin in some pathological conditions is still controversial. The aim of this study was to describe the use of albumin in patients with CKD and hypoalbuminemia, including the type, administration method, dose, frequency and correlation of therapy with outcome such as changes of plasma albumin concentration or clinical response and possibility of drug related problems (DRPs). It was an observational study with retrospective data collection based on medical records of patients that had been admitted to the inpatient ward of Dr. Ramelan Hospital from January 2014 to May 2015. This study included patients diagnosed with CKD and hypoalbuminemia and received albumin as therapy. This study has been approved by the hospital ethic committee. Total sample obtained was 63 patients, 43 patients were excluded from this study. All patients received 100 mL Albumin 20% (20 g), administered by infusion 6-8 drips/minute and not more than 4 hours. the frequency of albumin administration was 1 flask/week (85%), 2 flasks/week (10%), 3 flasks/week (5%). Mean plasma albumin level elevated by 0,3 + 0,1 g/dL post transfusion. No DRP was found in this study.
