Abstract
Efficaciousness of antimalarial medicine is critical to malarial control. Furthermore, the mutations in K13 propeller domain were strongly associated with artemisinin resistance. The proportion of patients who are parasitaemic on day 3 is the indicator for routine monitoring to identify suspected artemisinin resistance. Therefore, the objectives of this study are to primarily determine the proportion of day 3 positive in falciparum malaria patients treated with DHP in Papua, to discover mutations of K13 propeller, and to analyze whether risks are associated with delayed parasite clearance. Longitudinal study was conducted at Jayapura and Keerom, Papua in 2014. The efficacy of DHP was observed on day 1, 2, 3, 7, and 14 by microscopic examination and axillary temperature. The molecular examination to confirmed the P. falciparum infection and to identified K13 propeller domain using primer sets from codon 427 to 601 were performed by nested PCR and DNA sequencing. 99 patients of uncomplicated falciparum malaria were completed to follow-up. Asexual P. falciparum was found in 59%, 26%, 24% patients at day 3,7,14 respectively. We did not find mutations in the gene of K13 propeller. Logistic regression test showed that Papuan ethnics had 5.2 times higher risk for day 3 positive (95% CI: 1.68-16.1).
