Abstract

School health program (UKS) unit of health field activities in primary schools. This study aims to develop a database UKS of primary school level. The scope of this study include the implementation of UKS Trias and the system for recording and reporting on Implementation Team UKS primary school level. This type of research is Action Research. Subjects in this study is the implementation team of UKS primary school level I/240 Sutorejo Surabaya. Analysis of recording and reporting system of UKS State Primary School 1/240 Sutorejo found the problem that is source of data is available only in health services, so that the analysis can not be based Trias of UKS. This study resulted in a prototype database of UKS developed based on the analysis system for recording and reporting ongoing implementation team of UKS SDN 1/240 Sutorejo.The prototype database UKS that was developed to produce the information required by the implementation team UKS so that it can be used on internal management UKS in planning and evaluation of programs UKS.
