Abstract
The rapid epidemic growth of HIV is continuing in Indonesia. There are some factors which have influenced the spreading of this epidemic in Indonesia, such as the poor awarness to avoid unsafe free sex attitude and the sharing of needles and syringes among injecting drug users (IDUs). The sexual transmission of HIV has also apparently increased in Tulungagung. Commercial sex workers plays a significant role in the spread of HIV in Tulungagung. The many residents in Tulungagung have been worked at another country as a Indonesian migrant. This condition can cause the increase number of HIV-1 case and the possibility of genetic variation (subtype) HIV-1 in Tulungagung. This research is aimed to analyze the subtype distribution and to determine estimation of transmission mode on infected patient of HIV-1 and AIDS who came to Seruni clinic dr. Iskak hospital in Tulungagung. 40 patients with HIV infection and AIDS were interviewed to know the transmission mode. 40 plasma samples were examined by PCR to detect HIV RNA. The positive plasma samples of HIV-1 RNA were sequenced by using BigDye Terminator kit V1.1 cycle sequencing (ABIPRISMA 310 genetic analizer). The sequence result analysis was done by using Recombinant Identification Program (RIP) and phylogenetic trees (MEGA 5-2 soft ware). This research used 40 plasma samples from the infected patients of HIV and AIDS. HIV RNA was detected in14 of 40 plasma samples (35%). CRF01-AE subtype was found in 12 plasma samples which the sequence result was analyzed. The heterosexually transmission mode was found in 31 of 40 respondens subject (77,5%). The result number of negative PCR HIV achieved 26 of 40 plasma samples (65%), it happened because most subjects were getting ART. This result showed that CRF01_AE subtype and heterosexually transmission mode was found predominant among patients with HIV-1 infection and AIDS who came to Seruni clinic in dr. Iskak Hospital, Tulungagung.
