Abstract
Immunization is one of the health program which aims to reduce morbidity, disability and death from diseases that can be prevented by immunization (PD3I). in 2012, the data of infant immunization By Name By Address was processed by using Microsoft excel. Based on the problems identification at Tanjungsari health center, noted that the data of infant immunization By Name By Address was difficult to process to produce information as expected by Surabaya Health Department. This study aims to develop a database of basic immunization at Tanjungsari health center in order to facilitate the clinic staff in doing recording, processing and reporting the results of basic immunization. This research uses an action research approach in which the researcher developed a complete basic immunization database at Tanjungsari health center using epi info program. Renspondents of this study consisted of three immunization staff at Tanjungsari health center and one immunization staff who works in Surabaya Health Department. In the other hand, the test of new database development was done by one recording and reporting immunization staff at Tanjungsari health center. Based on the results of the development of basic immunization database, data processing can be done easier through the new database development, so that it can generate new informations, namely: frequency distribution of infants who were immunized by neighborhood, frequency distribution of infants who were immunized by where immunization took place and frequency distribution of infants who were immunized by vaccines. Informations about valid dose in giving basic immunization and drop out case in basic immunization by name by address were also obtained through the new program.
