Abstract

This research is to develop actions that surveillance DHF system from the old to the new surveillance DHF system. The purpose of this study was to develop a surveillance DHF data base that can provide information on eradication program DHFis complete, quick and informative in Surabaya City Health Department. This research was carried out on the section P2, P2 program DHF in Surabaya City Health Department in November 2015 up to July 2016. The informants are the head section P2, P2 program DHF manager, a data management officer DHF, program managers and program managers public health Tenggilis and Balas Klumrik. This type of data is primary data and secondary data obtained through observation and indepth interview. While secondary data obtained through the study of documents. Stage of development of database started from describing the surveillance system DBD ongoing in the Surabaya City Health Department, analyzing surveillance DHF in the Surabaya City Health Department to meet the needs of data and information, developing the base, test and evaluation of surveillance systems DHF developed. Design development of database starting from data normalization, ERD, and create the data dictionary. The next step to make the physical design using excel macros. The results of the evaluation of the development of database according to research informants implementation relatively easy data entry, processing the data into information said to be very easy, fast and get a more accurate result.
