Abstract

Nonadherence to hypertension treatment was reported as a major problem, leading to the increased incidence of cardiovascular diseases. A qualitative exploratory case study in four patients was designed to identify factors influencing treatment nonadherence for hypertensive patients as member of BPJS Health at a selected community health centre (Puskesmas) with the highest prevalence of hypertensive patients in Surabaya. Semistructure interview was collecting data method in this study that was used to four patients who were not beneficiaries of the contribution (non PBI) as BPJS Health member that were classified into BPJS Askes and BPJS Mandiri. Four patients were selected based on criteria such as age, blood pressure readings, gender and frequency of visits to the Puskesmas, as recorded in the Puskesmas’ system for information and management (SIMPUS). Thematic analysis showed that misperception to hypertension, forgetfulness to consume medicines and busyness were mostly identified as barriers to nonadherence. Lack of family support was reported a social factor that was influences patients’ adherence. Health system factors such as inadequate capacity of health service for all patients and limited number of medicines given to each patient was identified as treatment nonadherence barriers. Hypertension a chronic condition with no specific symptom was reported as a cause to patients’ nonadherence. Additionally, patients’ knowledge about BPJS program for patients with chronic conditions (Prolanis) was low. Therefore, rearrangement of national health system programs is a necessary to offer great benefit to hypertensive patients.
