Abstract

Diabetes mellitus (DM) is a disease that requires long-term treatment. Primary Health Care Centre provide DM medication to patients for several days. The number of days usually 5-10, so that the patient need to visit the centre more often to obtain their medicine. The short number of days is feared may affect patient adherence and cost. The objective of this research was to know the effect of number of days on prescription refill toward medication adherence and cost in DM patient at Primary Health Care Centre in Surabaya. This research was a cross sectional study using non-random sampling method. Adherence was measured using an Adherence to Refill Medication Scale (ARMS) questionnaire. Cost was obtained by interviewing DM patients regarding cost incurred in one month. Cost obtained including direct medical cost, direct non-medical cost and indirect cost. There were 89 patients from 14 Primary Health Care Centre in East Surabaya. There were 6 groups of days, mean value was 11.7 days, with the minimum of 3 days and maximum of 30 days. A low ARMS score indicates high adherence. The mean total score of ARMS was 16.55 ± SD 5.9 (Min score 12, max score 45). Spearman’s correlation between the number of days on prescription refill with ARMS score showed p value 0.039 and r -0.219. The total cost range from IDR 0 to IDR 221,952. Spearman’s correlation between the number of days on prescription refill with total cost showed p value 0.034 and r -0.225. There was a significant influence on the number of days on prescription refill to medication adherence and cost on DM patients at Primary Health Care Centre in East Surabaya. The longer the number of days on prescription refill, the patient is more comply yet the cost is lower.
