Abstract
Background: Hirschsprung disease can develop an suitable environment for pathogenic bacteria and others to grow and cause complication such as enterokolitis. Enterokolitis complication in hirschprung disease has a very serious mortality and morbidity. Objective This research is aimed to find complication of Hirschsprung disease, pathogenic bacteria and fungus that have been identified in stool culture and its antibiotic resistance. Method: This research is an observational – descriptive research. The data collection of this research is total sampling of Hirshcsprung disease patients in RSUD Dr.Soetomo Surabaya within 1stJanuary 2013 – 31stDecember 2015. Result: In this research there are 47 patients of Hirschsprung disease. Complication that most patient suffer are Malnutrition (68,75%), Enterokolitis (48.39%), and Sepsis (18.75%). Pathogenic bacteria that have been identified are E. coli (60%), Enterobacter cloacae (6.67%), Enterobacter aerogenes (6.67%), Raoultella ornithinolytica (6.67%), Klebsiella oxytoca (13.33%), Klebsiella pneumoniae (6.67%); and Pathogenic fungi that have been identified are Candida spp (61.54%), Candida famata (15.38%), Candida lusitaniae (7.69%), Cryptococcus humicola (15.38%). Antibiotic that is still susceptible for E. Coli are Piperacillin- tazobactam (100%), Ampicillin-sulbactam (50%), Cefoperazone-sulbactam (71%), Cefoxitin (67%), Ertapenem (83%), Imipenem (83%), Meropenem (89%), Tigercyclin (75%), Amikacin (89%), Fosfomicin (100%), Chloramphenicol (86%), Nitrofurantoin (100%); for Klebsiella oxytoca: Amoxicillin-clavulanate (50%), Ampicillin-sulbactam (50%), Cefoperazone sulbactam (100%), Cefepime (50%), Cefoxitin (50%), Aztreonam (50%), Ertapenem (100%), Imipenem (100%), Meropenem (100%), Trimethoprim-sulfamethoxazole (50%), Levofloxacin (100%), Ciprofloxacin (100%), Amikacin (50%), Fosfomicin (50%), Chloramphenicol (50%); for Enterobacter Cloacae: Piperacillin- tazobactam, Cefoperazone sulbactam, Cefepime, Cefotaxime, Aztreonam, Ertapenem, Levofloxacin, Ciprofloxacin, Tigercyclin, Amikacin, Fosfomicin, and Nitrofurantoin. ESBL in E. coli 33% and in Klebsiella oxytoca 50%. Conclussion: Pathogenic Bacteria and Fungi that have been identified in stool culture of Hirschsprung disease patient still caused entercolitis, sepsis and malnutrition and some of them even caused mortality.
