Abstract

Cardiovascular disease (CVD) is one of non-communicable diseases that cause high mortality in the world, contributing to 9,4 million total deaths annually. In Indonesia, 37% of total deaths caused by CVD. Hypertension is one of the main causes of high mortality and morbidity due to CVD in Indonesia. It was reported that one of the problems was about nonadherence to antihypertensive medicines amounted to 55,2%. According to previous studies, non-adherence to treatment of hypertension was influenced by six groups of factors, including patient factors, the disease factors, drug factors, factors of related to health personnel and health facilities, socio-cultural factors and health system factors. This study was conducted to: 1) determine influencing factors of non-adherence to treatment of hypertension for patients in a community pharmacy who were not covered by BPJS (non-BPJS), as well as 2) investigate the way how non-BPJS patients with hypertension obtained their treatment. Data were collected using semi-structured interviews with three purposively selected non-BPJS patients collecting antihypertensive medicines in a community pharmacy in Surabaya. All interviews were recorded and transcribed ad verbatim. Data were analysed using a thematic analysis method. The findings showed that nonadherence to treatment was influenced by the six groups of factors. Although the three non-BPJS hypertensive patients were found to be nonadherent to collecting their prescribed medicines in the studied pharmacy, they seemed to follow pharmacological and non-pharmacological recommendations. Despite that, two of the three informants were not adhere to hypertension treatment, while one informant was inversely. The three informants were recorded as nonadherent patients to make regular visit to the studied pharmacy because they usually collected their anti-hypertensive medicines in different health facilities.
