Abstract

Adherence to Antiretroviral Therapy (ART) is a gold way to achieve maximal suppression of HIV replication for PLWHA. It is to improving his health, It is to improving his health, improving his quality of life, and prolonging his survival. The adherence rate in Tulungagung district was low. People living with HIV-AIDS (PLWHA) who lost to follow up as much as 14.98%, PLWHA who still do ART as much as 54.31%, and PLWHA who did follow up with adherence rate more than 95% as much as 64.22%. The aim of this study was analysis the factors that influence adherence to antiretroviral (ARV) using the integrated behavioral model (IBM) theory approach. This study was observational analytic with case control design. Sampling was simple random sampling and obtained 99 people living with HIV-AIDS (PLWHA) into 2 groups. First group was 33 people who lost to follow up as control group. Second group was 66 people who did follow up with adherence rate more than 95%. The data was collected at VCT Seruni Clinic in RSUD dr. Iskak Tulungagung. This study also did home visit to collect primary data needed. This study used multiple logistic regression with α = 0.05. The result showed that the intention to perform the behavior of ARV adherence was influenced by attitude (p=0.006; Exp (B)=6.195), perceived norm (p=0.002; OR=8.903), and personal agency (p=0.002; Exp (B)=13,684). Then the adherence to antiretroviral (ARV) was influenced by knowledge and skills to perform the behavior (p=0.002 ; Exp (B)=19.596), salience of the behavior (p=0.002 ; Exp (B)=14.003), and environmental constraints (p=0.010 ; Exp (B)=21.637). However the intention to perform the behavior of ARV adherence did not influence the adherence to ARV. The behavior of ART adherence did not affected by the intention of PLWHA to adherence with ARV. Environmental contrains are the most powerful factor influencing behavior of ARV adherence. Family Support and peer support will help increase self-efficacy of PLWHA in facing various environmental constrains that arise to ARV adherence. So it is expected that PLWHA can open their HIV status to their family and participate actively in peer support groups activities.
