Abstract
Background : Methotrexate has distinctive characteristics compared to other cancer drugs, namely methotrexate has a very wide dose range (3 mg–33600 mg/m2) and requires monitoring of methotrexate levels. Measurement of methotrexate level is important in the use of high dose methotrexate to identify patient with a risk of toxicity, such as decreased of renal function. Objectives : To analyze the use of high-dose methotrexate on renal function during chemotherapy consolidation phase in pediatric patients with ALL. Method : An observational prospective study, total sampling method was conducted for 2 months (March-April 2019) in the Pediatric Department of Dr. Soetomo Teaching Hospital Surabaya. Patients who met the inclusion criteria were given high dose methotrexate according to the 2018 Indonesian ALL Chemotherapy Protocol. Measurement of methotrexate level and serum creatinine level was done 3 times on each cycle of chemotherapy consolidation phase. Measurements were made on the 0, 36, and 90 hours after prehydration. This study had been reviewed by Ethics Committee Dr. Soetomo Teaching Hospital Surabaya. Results : There were 13 patients with 19 cycles who met inclusion criteria. There was no significant change on renal function after the administration of high dose methotrexate (p>0.05). Conclusion : High-dose methotrexate does not cause a decrease on renal function in pediatric patients with ALL in Indonesia.
