Abstract

Tuberculous meningoencephalitis occurs when Myobacterium tuberculosis invade the membranes and fluid surrounding the brain and spinal cord. The infection usually begins elsewhere in the body, usually in the lungs, and then travels through the bloodstream to the meninges. The purpose of the study was to analyze the usage of antibiotic and tuberculostatic on tuberculous meningoencephalitis in Department. Neurology Dr.Soetomo Teaching Hospital Surabaya January 1,2008 until-December 31, 2010.The study was retrospective using data from Medical Record and was analyzed descriptively.The result showed that most of tuberculous meningoencephalitis occured on patient in average of 15-60 year (50%) and was dominated by male (75%). All the patient showed the symptoms and signs of Tuberculous meningoencephalitis were stiff neck, fever, decreased consciousness, and headache. The complication diseases which often occured during the treatment were hydrochephalus (35%) and septic shock (35%). The secondary disease was tuberculous pulmonary, suffered by all the patients. All the patients received intravenous ceftriaxone 75-100 mg/kg/day for empiric therapy. Intravenous fosfomycin 1-2g/kg/12 hours was used as prophylaxis in one VP Shunt patient. All the patiens received oral isoniazid 5-10/mg/kg/day , oral rifampicin 15-20/mg/kg/day , oral pyrazinamide 30-35 mg/kg/day , and intramuscular streptomycin sulfat 15 mg.kg/day. Oral ethambutol 15-20/mg/kg/ day was used as alternative therapy for two hepatotoxic patients. Drug Related Problems which happened was interaction between isoniazide and rifampicin caused hepatotoxicity increased (15% patients)
