Abstract

The aim of this study was to identify utilization profile of intermediate-acting and long-acting insulin in hospitalized diabetes mellitus patients in Dr.Soetomo general hospital Surabaya and problem related to the use of insulin. The study was conducted prospectively from March- May 2012. The inclusion criteria was diabetes mellitus patient, type I or type II with or without complication who received intermediate-acting or long-acting insulin, with complete medical record. The result showed that from 55 patients, most of them in the range of age 45-64 y.o (74.55%). The most complication and comorbid was foot ulcer (56.36%) and urinary tract infection (32.73%). Condition of infections occured in most patients with diabetes mellitus (74.55%) that could influence blood glucose control. The type of intermediate-acting insulin used was Humulin N (18.18%) whereas long-acting insulin was detemir (61.82%) and glargine (20.00%). Long-acting insulin was preferred than intermediate-acting insulin related to secretion of long-acting insulin closely mimic secretion insulin physiologic and the lower risk of nocturnal hypoglycaemia. The initial and maintenance dose of intermediate-acting and long-acting insulin could be varied due to FPG/ random blood glucose and patient’s condition. The incidence of DRPs identified in this study were side effects from the use of insulin i.e. hypoglycaemia (1.82%) and hypokalemia (3.64%), interaction between insulin with other drugs, and alteration dose of insulin without record blood glucose (10.00%).
