Abstract

Methanol intoxication is an acute medical emergency. It can lead to considerable morbidity as well as mortality. In cases of methanol poisoning, formic acid as a metabolic toxic accumulates and triggers the incidence of metabolic acidosis. Ethanol, Sodium Bicarbonate, and Folic Acid are used for methanol intoxication. Ethanol competes with methanol for the enzyme alcohol dehidrogenase which has a greater affinity to ethanol than methanol. Sodium Bicarbonate is administered to patients with significant metabolic acidosis; whereas Folic acid increase metabolism of formic acid. In addition, hemodialysis must be started in patients with severe metabolic acidosis to increase the clearance offormic acid. The research was a retrospective study. It retrieved data from patient medical records from January 1st , 2010 until December 31 st, 2014. The data were then compared to guidelines of methanol intoxication. The results showed that methanol intoxication prevalence was mostly seen in male (96%) and aged 21-30 years old. The severity of poisoning had been graded as mild (3 patients), moderate (12 patients), and severe (11 patients). The antidotes used were Ethanol, Sodium Bicarbonate, and Folic Acid. The prognosis was correlated to the degree of metabolic acidosis. The conclusion antidotes used were Ethanol, Sodium Bicarbonate, and Folic Acid which dose regimentation, route of administration, and duration were similar to the three staging of severity (mild, moderate, and severe). However, the observed differences were the frequency and combination of antidotes and route of administration. And drug-related problem of Ethanol was hypoglycemia as adverse drug reaction.
