Abstract

Atrial Fibrillation is the most common cardiac dysrhytmia characterized by a very rapid, chaotic electrical activity of the atria, resulting in accelerated and irregular ventricular activity. AF is associated with an increased risk of tromboembolism which leading to stroke. Warfarin is an oral anticoagulant to prevention and treatment of tromboembolism in patients with atrial fibrillation. It has a narrow therapeutic index, wide inter-individual dosage variability, and interaction with many drugs and foods. This study was designed to evaluate the usage of warfarin in Atrial Fibrillation based on right indication, right patient, right drug, right dose, and of adverse drug reaction awareness. This study was conducted retrospectively from 1st Januari to 31st December 2012. A total of 26 patient was diagnosed AF and receive oral anticoagulant warfarin and have no containdication. Sample of this study are 10 males and 16 females with an age range of 26-39 years (50%), 40-59 years (38%), and 60-74 years (12%). In this study found 3 patients (12%) have beseline INR, 19 patients (73%) have INR value, and 12 patient (46%) reach INR target 2-3. These 12 patient consist of 7 warfarin naive and 5 history of warfarin. An adverse drug reaction found in patient with INR value 6,10 which have gums bleeding then receive intravenous vitamin K 10 mg/1 ml to stop the bleeding. Keyword.
